Family Nurse Practitioner Outpatient Community Health Minersville, PA

A respected and mission-driven Federally Qualified Health Center (FQHC) one of the largest community-based networks in Pennsylvania is seeking a Family Nurse Practitioner to join its outpatient clinic in Minersville, PA. • Outpatient-only primary care practice

• Full-time, 40-hour work week, including 35 hours of direct patient care

• Clinic hours: Monday Friday, 8:30 AM 5:00 PM, with one weekly evening shift until 7:00 8:00 PM

• PCMH model collaborative, team-based care approach

• Full-scope Family Medicine, treating patients of all ages

• Visit durations:

  • New patients: minutes

  • Follow-ups/med management: minutes

• Supportive environment with experienced physicians, behavioral health integration, and nursing staff
