abuse. Horizon House is Pennsylvania’s oldest psychiatric
rehabilitation agency. Since 1952, Horizon House has been dedicated
to helping people with mental illness, drug and alcohol addictions,
developmental/intellectual disabilities, and homelessness to
stabilize their lives and to assist them to live as independently
as possible in a community-based setting. With over 100 locations
throughout Southeastern Pennsylvania and the state of Delaware,
Horizon House provides services to a diverse population focusing on
individual strength and choice. We are seeking a qualified
candidate interested in joining our Residential Service Group Home
team at our Wilson Road location in Wilmington, Delaware, 19803.
Work Schedule: Monday through Friday, 40 hours workweek, and 8
hours shifts Hire requirements for this opportunity include
attending new-hire orientation during the first five (5) workdays
(Mon–Fri), at our Philadelphia location. Mileage reimbursement is
provided. The Residential Associate Clinician will provide
coordination and guidance, counseling, and encouragement to
participants as they transition from residential services to a more
independent environment. The Residential Associate Clinician will
also implement and document participant’s progress in accordance
with individualized rehabilitation plans and ensure a safe, healthy
and secure environment which promotes growth and development. Some
additional responsibilities would include the following: Complete
the initial and annual participant assessments and develop the
participant’s recovery plan from a person-centered perspective.
Assist with the presentation in case conference for assigned
participants. Responsible for the care and maintenance of case
records for all assigned participants. Act as a liaison and consult
with community agencies and families to maintain coordination in
the recovery process. Provide ongoing assessment of participant’s
mental illness symptoms and their responses to recovery to ensure
that immediate and appropriate interventions are provided.
Participate in staff planning, training and internal/external
administrative meetings. Qualifications: Bachelor’s degree in
counseling, psychology, social work, nursing,
vocational/psychiatric rehabilitation, education or other mental
health field from an accredited college or university. Knowledge of
mental health and substance abuse systems and supports. Ability to
work independently. Good communication skills both oral and
written. Possess a valid driver’s license with an acceptable
driving record. Direct experience in mental health services with
emphasis on direct care in a community-based setting. Scope:
Requires the ability to lift, carry, fold, bend, kneel, stack,
stoop, which includes direct intervention with participants.
Frequent talking and listening. Ability to climb and descend
stairs. Possess manual dexterity and fine motor skills. Requires
crisis intervention in emergency situations. May be required to
report to work during emergencies including inclement weather.
